HIV / AIDS Program


Red AIDS Ribbon

Queen's HIV/AIDS Community Care Program specializes in providing in-home services for HIV/AIDS clients to improve their quality of life and delay or prevent unnecessary institutionalization. Monthly contact by a registered nurse and/or social worker are done to help coordinate services for clients.

If you're interested in applying to Queen's HIV Program, please feel free to print, and complete both the application and release of information forms available for download on the right side of this page. Return the completed forms to Queen's via FAX or regular mail. HIV services include:

  • Case management
  • Home delivered meals
  • Skilled nursing care
  • Chore services
  • Counseling and training
  • Emergency alarm response service
  • Home health aides
  • Non-medical transportation
